• Proof of residence (utility bills, mortgage, or lease agreement—Fill this form out if you cannot provide either) The DOR will need to be notarized.

• Immunizations (exemptions available through your provider)

• Birth certificate

• Parent ID for all parents listed on Birth Certificate.

• Oral Assessment for Incoming Kindergarteners only: (download the form here)

Please do not upload pictures of Documents. You can use the app Genius Scan to scan documents if needed. Please make sure scans are clear and capture the full page.
